Matt Guy will join Beth Katzenberg and a panel of clinic professionals to discuss Improving Communication in Transitions of Care and Reducing Readmissions. This panel is scheduled for Thursday April 4th from 11:00-12:00. Improving Communication and Readmissions (iCARE), a CRHC initiative, began two years ago with Colorado’s Critical Access Hospitals looking at improving communication in transitions of care, reducing avoidable readmissions, and improving clinical processes that contribute to readmissions – all important state and national topics. Now in its 3rd year, the program has expanded to include hospital’s provider-based rural clinics as we broaden the spectrum of these goals and examine the important role clinics play and the impact clinic/hospital communication and processes can have in these areas. This session will provide a brief overview of the iCARE project, the work done to-date, lessons learned, as well as experiences of participants.
Matt Guy is the Executive Director of the Southeastern Colorado Area Health Education Center (SECAHEC). SECAHEC serves 16 counties in southeastern Colorado providing health education resources and support for current and future medical professionals.

Matt has also worked for the Colorado Rural Health Center working with safety net and rural health clinics in Southeastern Colorado to improve the quality and effectiveness of their practices, including offering assistance with National Committee for Quality Assurance’s Patient-Centered Medical Home recognition program.
