Colorado Colorectal Screening Program to Exhibit at the Forum!!

The Colorado Colorectal Screening Program will exhibit at the Forum.
The program, which began in January of 2006, sought to screen over 11,400 Coloradans by the end of 2010 for colorectal cancer, contributing substantially towards the state’s overall goal of screening 75 percent of the total eligible population. The program is accompanied by an awareness campaign to encourage all Coloradans, particularly those uninsured and income eligible,  to get colorectal screening exams.
The Program, coordinated through the University of Colorado Cancer Center, is currently based in 49 community clinics and partners with more than 250 screening, pathology, and treatment providers.  Through May 2012, the Program has completed more than 13,890 endoscopic screens and detected 118 cancers.

Matt Guy to join Beth Katzenberg and others on a breakout session panel!

Matt Guy will join Beth Katzenberg and a panel of clinic professionals to discuss Improving Communication in Transitions of Care and Reducing Readmissions. This panel is scheduled for Thursday April 4th from 11:00-12:00. Improving Communication and Readmissions (iCARE), a CRHC initiative, began two years ago with Colorado’s Critical Access Hospitals looking at improving communication in transitions of care, reducing avoidable readmissions, and improving clinical processes that contribute to readmissions – all important state and national topics. Now in its 3rd year, the program has expanded to include hospital’s provider-based rural clinics as we broaden the spectrum of these goals and examine the important role clinics play and the impact clinic/hospital communication and processes can have in these areas. This session will provide a brief overview of the iCARE project, the work done to-date, lessons learned, as well as experiences of participants.

Matt Guy is the Executive Director of the Southeastern Colorado Area Health Education Center (SECAHEC). SECAHEC serves 16 counties in southeastern Colorado providing health education resources and support for current and future medical professionals.

Sue Hansen to discuss Culture Change

Getting into the Groove: positioning an organization for positive change

Transforming the culture of an organization is akin to herding cats. It’s hard enough to succeed at changing anything in an organization, much less trying to change the entire “spirit” of the place! Culture is so palpable in an organization that a new applicant can see it soon after being hired and sometimes on the very same day! Culture reflects the shared attitudes, the values, goals and practices of a group of people. Culture reflects the customary beliefs, social contracts, rules of behaving, how we speak, how we interact. Culture is a mirror-if you want to see the culture of your organization, one just needs to look in the mirror. Discover how to position your organization for change, lead the change and most importantly have a sense of humor during the change!

Thursday April 4th
12:30-1:30 - General Session
